Dear Patients,
Allergy and Asthma Medical PC is moving to a new location. This is very exciting news for us, as the new office location is midway between our two current offices, and will allow for centralized records and operations, and improved patient communications and care.
The new location is located at 200 W 57 th St, 15 th floor (on the corner at 57 th and 7 th Avenue), and is a beautiful state-of-the-art facility that has a large outdoor terrace and views of Central Park. This office location will be the new flagship outpatient group practice of St. Luke's/Roosevelt Hospital, and primary doctors and specialists of all types will practice alongside Dr. Horne at this location.
The details are:
Dr. Horne will no longer see patients at the Miller Practice on 52 nd st. as of January 1st, 2011. The month of January will be a transition month as we transfer our records to a new electronic medical record and the buildout of the new space is completed.
During January, Dr. Horne will continue to see patients at his 1886 Broadway location (Broadway and 62 nd st.) on Monday 1-7pm, Wednesday 5-7pm and Thursday 8am-12pm. The phone number of the 1886 Broadway office (212 247-2080) will continue to be used as the phone number of the new office location.
The new space is slated to begin operations on February 1, 2011, and the office hours will be greatly expanded from the ones at our current locations. These hours will be posted in January on our website.
For allergy shot patients from the Miller Practice, your serum and records will be moved to 1886 Broadway for the month of January, and then to the new location. We apologize for the inconvenience, and Dr. Horne will work to adjust your dose to account for any disruption in your schedule. Please note the evening office hours (M and W up to 7 PM) available in January at the 1886 Broadway location, If you prefer to have your shots or records transferred to another practitioner in the area, please let us know and we will assist in this transfer, but of course we hope that you stay with us!
We are very much looking forward to seeing you at our new location, as we anticipate big improvements in patient communications with new software and increased availability via expanded office hours.
